Naftalan for Arthritis and Joint Pain: What the Evidence Shows and Who Should Avoid It (2026)

explainer · 8 min read

By Emin Abdulalimov

Naftalan oil is traditionally used for inflammatory musculoskeletal and rheumatic joint pain in Azerbaijan, but the evidence is observational, not from randomized trials. This guide separates what's actually supported from resort marketing, states the full contraindication list, and names the sanatoriums running a joint-focused course.

Naftalan oil is traditionally used for inflammatory musculoskeletal and rheumatic joint pain at sanatoriums in Naftalan, Azerbaijan, based on uncontrolled clinical experience, not a randomized trial. A Croatian Medical Journal review covering roughly 14 years of that experience concluded the therapy "should be further investigated." Naftalan is contraindicated in pregnancy, active cancer, and several other conditions.

Birtour is a Baku-based DMC that books stays at Naftalan's sanatoriums; we are not a medical provider, and we do not operate any of the properties named on this page. Nothing here is medical advice. Suitability, contraindications, and outcomes are set by each venue's clinical team, not by us; confirm with a physician before booking a course. Prices below are indicative and dated; ask us for a current quote.

Does naftalan help arthritis or joint pain?

Naftalan oil is traditionally used for, and reported to help with, inflammatory musculoskeletal and rheumatic joint pain, the same broad evidence class as its use for psoriasis and other dermatoses. No study has shown it treats or cures arthritis or joint disease. What exists is decades of uncontrolled clinical experience at Naftalan plus a published review, not a controlled trial against a placebo or a standard treatment.

Naftalan does have one double-blind randomized controlled trial to its name, but it treated oral lichen planus and recurrent aphthous stomatitis, a mouth condition, not arthritis or joint disease, and found no significant efficacy difference against the steroid betamethasone. Citing that trial as evidence naftalan works for joint pain is a citation error; the trial that exists answers a different question.

Naftalan for rheumatoid arthritis and osteoarthritis: what the evidence shows

The naftalan literature does not separate rheumatoid arthritis from osteoarthritis. Both fall under one heading, inflammatory musculoskeletal and rheumatic pain, in the review evidence, with no controlled comparison between the two conditions or against a placebo. Treat any claim that naftalan has been specifically studied in rheumatoid arthritis, or specifically in osteoarthritis, as unsupported; the observational base does not go down to that level of diagnosis.

Rheumatoid arthritis is an autoimmune, inflammatory joint disease. Osteoarthritis is a degenerative one with a different mechanism. A sanatorium may market naftalan for both under the same rheumatic-pain umbrella, but we have not found a study that reports outcomes separately by diagnosis, disease activity, or medication regimen. If you manage rheumatoid arthritis with disease-modifying or biologic therapy, that is exactly the detail to raise with a physician before booking, not after arrival.

What is the best Azerbaijan sanatorium for joint pain?

We do not rank sanatoriums; the venue's clinical team decides suitability once you arrive. Nafta Sanatorium in the town of Naftalan positions itself specifically around psoriasis and rheumatic conditions and is rated 9.2 on Booking.com, the clearest joint-pain positioning among the properties we have reviewed.

Gashalti Health Hotel and Chinar Health Hotel & Spa also run naftalan courses that pair with physiotherapy for musculoskeletal complaints, though their marketing leans more toward dermatology and general wellness than joint pain specifically. What we can state with confidence is positioning and pricing, sourced to each property's own listing; outcomes and final suitability stay the sanatorium's clinical call.

Roughly 14 properties operate in the town as of July 2026, per naftalanhotels.com's own active-property list. The four above are the most established for a joint-focused course; smaller properties exist, but we have not verified their rheumatology-specific protocols independently.

How does a naftalan course work for joints?

A naftalan course for joint pain pairs warm-oil immersion baths, 8 to 15 minutes at 34 to 38°C, with physiotherapy, sold in 7, 10, or 14-day blocks, 14 days standard at most Naftalan properties. Package composition varies: dermatology-focused courses commonly add UVA or UVB light, which may or may not be part of a joint-focused booking, so confirm the exact package with the sanatorium before you travel.

The proposed mechanism for naftalan's effect on joints has not been established in the literature we reviewed. Most mechanistic study of the oil, in-vitro keratinocyte work and its naphthenic-acid content, comes from dermatology research, not rheumatology. Sanatorium staff may describe a firmer mechanism than the published evidence supports; treat mechanism explanations as a working theory, not settled science, the same caution that applies to naftalan's use for psoriasis.

When is naftalan not safe?

Naftalan is contraindicated in pregnancy and lactation, active cancer, acute febrile infection, stage 2 to 3 hypertension or severe cardiac disease, bleeding disorders, and known allergy to petroleum products. Screen with a physician before booking, not after arrival, and disclose any joint-specific medication, including disease-modifying or biologic therapy for rheumatoid arthritis, at the time you enquire.

  • Petroleum allergy. Naftalan oil is a petroleum derivative; a known allergy to petroleum products rules it out.
  • Pregnancy and lactation. Excluded across every source we checked, with no exception noted.
  • Active cancer or tumors. Listed as a contraindication in the dermatology literature we reviewed for naftalan generally; no joint-pain-specific exception is documented.
  • Acute febrile infection. Wait for the infection to clear before booking a course.
  • Stage 2 to 3 hypertension or severe cardiac disease. Warm immersion raises cardiovascular load; screen with a cardiologist first.
  • Bleeding disorders. A prudent additional caution for hot immersion bathing; confirm with a physician.

A separate safety question hangs over the oil itself, apart from the joint condition being treated. Refined "white" naftalan was tested in a 1978 University of Zagreb trial on 770 patients and found not carcinogenic, but the crude-oil immersion baths that make up most joint-pain courses in Azerbaijan are a different formulation, one the trial never examined. Naphthalene is a distinct chemical compound that the International Agency for Research on Cancer places in Group 2B, "possibly carcinogenic to humans"; that classification belongs to naphthalene, not to naftalan oil, though the two names get mixed up often enough to cause confusion. Full international medical certification for naftalan oil remains underway rather than granted as of mid-2026, so any claim that a joint-pain course carries regulator approval is premature.

What we don't claim

  • We don't call naftalan a cure for arthritis or joint pain. The evidence supports "traditionally used for" and "reported to help" language, not "treats" or "cures."
  • We don't cite the naftalan RCT as joint-pain evidence. That trial treated oral lichen planus; using it to back an arthritis claim is a citation error we won't make.
  • We don't split rheumatoid arthritis from osteoarthritis ourselves. The published literature reports them together under one heading; we report that boundary rather than inventing a finer breakdown.
  • We don't quote a single all-in medical price. Naftalan programs bundle room rate and treatment differently by property; we send a current quote once we know your dates and any medical restrictions.
  • We don't handle the clinical side. Diagnosis, disease staging, and course design are the sanatorium's clinical team's job, not ours.

For the full evidence and safety picture across all naftalan indications, not just joint pain, see our naftalan oil therapy guide. For operators or families ready to check dates, our DMC quote request guide has the template that gets a fast, accurate answer.

Frequently asked
Does naftalan help arthritis or joint pain?
Naftalan oil is traditionally used for inflammatory musculoskeletal and rheumatic joint pain, based on uncontrolled clinical experience built up in Azerbaijan, not a randomized trial. The one naftalan RCT that exists treated oral lichen planus, not joint disease, and found no advantage over a standard steroid. Any claim citing that trial as arthritis evidence is a citation error.
Does naftalan work differently for rheumatoid arthritis than for osteoarthritis?
No. The observational literature groups naftalan's joint-pain evidence under one heading, inflammatory musculoskeletal and rheumatic pain, without separately reporting outcomes for rheumatoid arthritis or osteoarthritis. A Croatian Medical Journal review covering roughly 14 years of clinical experience concluded the therapy 'should be further investigated,' the same uncontrolled evidence class as naftalan's psoriasis use.
