Naftalan Oil Therapy in Azerbaijan: What It Treats, the Evidence, and Who Should Avoid It (2026)

Naftalan oil is a heavy naphthenic crude oil found at the town of Naftalan, about 330 km west of Baku, Azerbaijan. Warmed to 34 to 38°C, it is used in immersion baths for psoriasis and inflammatory musculoskeletal pain. Azerbaijan and Ivanić-Grad, Croatia, are the only two places in the world offering this therapy.

Why Naftalan, and where our job stops

Naftalan sits 330 km west of Baku, has roughly fourteen trading sanatoriums, and has no direct coach from the capital. It is a transfer problem and a room-availability problem before it is anything else, and those two are our job. We book the stay, arrange the ground, and quote it; the clinical decision is between you and your own physician.

Most enquiries that reach us are families rather than tour operators, and most of them arrive having read three sanatorium brochures that say the same seven things. What they do not have is the boring layer: which property the state actually rates, which one publishes its room count, how you cover the last 50 km from Ganja, and whether the flight schedule they found online is nine years old. What is naftalan oil?

Naftalan oil is a heavy naphthenic crude oil pumped near the town of Naftalan in western Azerbaijan, about 330 km from Baku. It is about half cycloalkanes, with aromatics, resins, and naphthenic acids making up the rest. Therapeutic bathing in the oil dates to the early 1900s, and a dedicated resort has operated at Naftalan since 1933.

During the Soviet era the resort drew roughly 75,000 visitors a year. Naftalan oil is not unique to Azerbaijan, whatever some resort marketing implies: it is used therapeutically in exactly two places on earth, Naftalan and the Special Hospital Naftalan in Ivanić-Grad, Croatia, which uses a refined derivative of the same oil rather than the Azerbaijani crude. Azerbaijan's government has backed Naftalan as its flagship medical-tourism brand; a September 2024 presidential decree specifically targets increasing the role of Naftalan oil in medical tourism. As of August 2026 booking agents list 14 trading properties in the town (naftalan-booking.com), and Deputy Health Minister Nadir Zeynalov said in October 2024 that roughly half of Azerbaijan's health and wellness facilities sit in this one city. We have not found a more recent count, so treat that as a 2024 figure.

Naftalan oil treatment: what the evidence actually supports

Evidence-supported naftalan oil treatment covers two areas: psoriasis and other dermatoses during remission, and inflammatory musculoskeletal or rheumatic pain. Both rest on uncontrolled clinical experience and small observational studies, not randomized trials. The commonly marketed claim that naftalan treats 70-plus or 75-plus diseases, spanning gynecology, urology, neurology, and infertility, has no trial evidence behind it.

Naftalan does have one randomized controlled trial to its name, but it is easy to miscite. The double-blind RCT identified in the literature treated oral lichen planus and recurrent aphthous stomatitis, not psoriasis or joint disease, and found no significant efficacy difference against a standard steroid, betamethasone. Treat any claim that an RCT proves naftalan treats psoriasis as wrong; the trial that exists answers a different question.

How does naftalan therapy work?

A naftalan course means warm-oil immersion baths, 8 to 15 minutes at 34 to 38°C, usually followed by 15 to 20 minutes of UVA or UVB light and a physiotherapy session. Sanatoriums sell courses in 7, 10, or 14-day blocks, with 14 days the standard length at most properties.

The proposed mechanism draws on in-vitro keratinocyte studies and the oil's naphthenic-acid content, not on a confirmed clinical pathway; sanatorium staff will often describe it in more confident terms than the trial literature supports. A typical booking pairs the daily bath course with full board and other balneotherapy add-ons, such as mud or mineral-water treatments, sold as a package rather than a stand-alone bath.

One property publishes a block outside that pattern. Karvan Hotel-Sanatorium lists course length as 7, 10, 14 or 21 days and states the choice is made with its doctors, that its bath is held at 37°C, and that each session lasts exactly 10 minutes. We list the 21-day option because it exists on a live booking page, not as a recommendation. Length is the venue's clinical call and your physician's, never ours.

Is naftalan oil safe?

Naftalan oil therapy carries a manageable safety profile for screened patients, but the safety data has a gap that matters: it covers refined "white" naftalan, not the crude oil most Azerbaijani resorts use in immersion baths. A 1978 University of Zagreb trial on 770 patients found the refined distillate was not carcinogenic; that finding does not automatically extend to crude baths.

Naphthalene, the chemical compound, is classified by the International Agency for Research on Cancer as Group 2B, "possibly carcinogenic to humans." Naftalan oil is a different substance, a complex crude oil rather than a single compound, and the two get confused in casual write-ups. Neither the naphthalene classification nor the 1978 refined-oil study settles the safety question for the crude immersion baths most Naftalan properties run; we have not found a published study that fills that gap, and we would not claim otherwise.

Naftalan oil also has not received full international medical certification as of mid-2026; Azerbaijani authorities have described certification as underway, not complete. Treat any claim that Naftalan therapy is internationally regulator-approved as premature.

Naftalan contraindications: who should not book a course

Naftalan therapy is not appropriate for everyone. Confirmed contraindications include allergy to naftalan or other petroleum products, pregnancy and lactation, active cancer, acute febrile infection, stage 2 to 3 hypertension or severe cardiac disease, and bleeding disorders. Psoriasis patients should book only during remission; the acute, flaring stage is a contraindication, not an indication.

• Petroleum allergy. Naftalan oil is a petroleum derivative; a known allergy to petroleum products rules it out.
• Pregnancy and lactation. Excluded across every source we checked, with no exception noted.
• Active cancer or tumors. Listed as a contraindication in the dermatology literature.
• Acute febrile infection. Wait for the infection to clear before booking a course.
• Stage 2 to 3 hypertension or severe cardiac disease. Warm immersion raises cardiovascular load; screen with a cardiologist first.
• Bleeding disorders. A prudent additional caution for hot immersion bathing; confirm with a physician.
• Psoriasis during an active flare. Indicated only in the stationary or remission stage; contraindicated during the acute, progressive phase.

Every sourced reference we checked tells the same story: screen with a physician before booking, not after arrival. Minimum age for children varies by source, three years in some references and five in others; we state it conservatively and recommend confirming the specific sanatorium's policy for children under five. None of this replaces a conversation with your own doctor, particularly if you take blood thinners or manage a chronic cardiac or renal condition. Naftalan sanatoriums operating in Azerbaijan (2026)

Booking agents listed 14 trading properties in Naftalan in August 2026: Chinar, Gashalti, Garabag Resort & Spa, Gold, Sun City, Park, Nur, Karvan, Kepez, Gozel, Nafta, Naftalan Central, Miracle Naftalan and the NAFTALAN Sanatorium. The table below covers the four most established. Rates are the properties' own listed per-night figures, indicative and dated, not net B2B pricing.

Four properties publish enough about themselves to be checked. The rest do not: Chinar, Garabagh and Gashalti all run client-rendered sites that serve no readable text, so their room counts and opening dates circulate through aggregators only. We treat an aggregator room count as a rumour.

We could not date a single opening, closure, change of hands or refurbishment in Naftalan during 2025 or 2026. Karvan's "completely transformed in 2022" is the only renovation any property puts a year on. Anyone quoting you a "newly renovated 2026" sanatorium is quoting a brochure, and there is a 2019 development article that gets recycled as current more often than it should be.

What the state star register says about Naftalan hotels

Azerbaijan's Tourism Board publishes a live register of star-classified hotels. Seven Naftalan properties hold an official category: Chinar and Garabagh at 4 stars, Gashalti, Gold and Sun City at 3, Nur and Park at 2. Booking agents sell several of the same properties one or two stars higher. The register is the one that was audited.

Star classification is not optional. Under Article 9.1 of the Law on Tourism a hotel has six months from opening to obtain a category, and trading without one draws a 2,000 to 5,000 AZN fine under Article 428-1.2 of the Code of Administrative Offences. The audit is run by the Certification Body for Hotel Star Categories at the Azerbaijan Tourism Board, accredited to AZS ISO/IEC 17065:2021, against criteria the State Tourism Agency Collegium approved on 25 May 2026.

Absence from the register is not a black mark. Several Naftalan houses are registered as sanatoriums rather than hotels, a different legal category with a different regulator. What it does mean is that where a property is off the register, the star on the brochure was not issued by anybody. Judge those on their treatment base and their room, not on a number a marketing team chose.

Who owns the Naftalan sanatoriums?

One Naftalan operator is confirmed non-private: the Kurort Naftalan Health Center on Heydar Aliyev Avenue 198, 50 beds in 26 rooms, belonging to Kurort CJSC of the Azerbaijan Trade Unions Confederation. The ownership of the other thirteen trading properties is not published anywhere we could find. Assume private unless the property tells you otherwise.

The union side books differently, and that is worth knowing when a property tells you it is full. AHIK issues sanatorium vouchers centrally through its Kurort tourist information centre and an e-AHIK reservation system, and publishes its own Kurort Naftalan rates: 180 AZN for the 18 m² economy single, 180 to 220 AZN for a 26 m² standard, 200 to 240 AZN for a 40 m² senior suite. That is a different counter from the one an OTA talks to.

Private medical activity in Azerbaijan is a licensed activity, item 2 of Appendix 1 to Law No. 176-VQ of 15 March 2016. The Ministry of Economy issues the licence on a positive opinion from the Ministry of Health, which then handles monitoring. A property running treatment should be able to give you its licence number without going quiet. If asking makes the conversation awkward, that is information.

How do you get to Naftalan from Baku?

There is no direct Baku to Naftalan coach on the bus terminal's route list. You go via Ganja, 360 km and 13.10 to 13.50 AZN by coach with departures roughly every half hour through the afternoon, then 50 km on; or by rail to Goran station, 20 km out. For a treatment stay we book a private transfer.

Two warnings on published timings. The bus terminal prints "2 hour 5 minute" for the 360 km Baku to Ganja run, which is not a journey any vehicle makes; treat avtovagzal.az distances and fares as sound and its times as decoration. And on 11 and 12 August 2026 the Heydar Aliyev departures board showed Nakhchivan as the only domestic destination. The "Baku to Ganja Monday, Wednesday, Friday and Sunday at 11:05 for 56 manat" schedule that circulates on travel sites traces back to a January 2017 article. Do not build an itinerary on it.

The one real access change since 2025 is on rail: Azerbaijan Railways launched daily Ganja to Gabala service on 12 January 2026, 139 km in 1 hour 50 minutes from 4.30 AZN, and Goran is a stop on it. Our Naftalan and Gabala access guide has the full routing for anyone combining both towns.

What to ask your own doctor before you book

Azerbaijan will not screen you at the border. No state portal or property site we checked lists a doctor's referral, a medical certificate, a vaccination record or health insurance as a condition of booking a naftalan stay. That absence is not a rule saying none exists, but in practice the screening this therapy needs happens in your own doctor's room, before you pay a deposit.

• "Given the contraindication list on this page, is there anything in my history that rules this out?" Print the list above and hand it over rather than paraphrasing it.
• "Do any of my current medications matter for warm immersion?" Bring the real list, including anything over the counter.
• "If I have psoriasis, what stage am I in right now?" The venue will ask, and the page above explains why the answer changes the booking.
• "Is a 10 to 14-day absence from my usual care workable?" Courses are sold in blocks, and the block is the trip.
• "What would you want to know if something changed while I was away?" Agree the follow-up before you travel, not after.

Karvan states that no Naftalan bath begins without a prior on-site medical consultation and an individual programme. That screen is real, and it is the pattern across the town's better properties. It also happens after you have paid and travelled, which is why it is not a substitute for the one before you book.

What we would not recommend

Five things we argue against, in the order they come up. None of them is a clinical judgement; they are booking judgements, made from quoting this town and covering the road. Where the question is medical, we say so and send it back to your physician rather than answering it ourselves.

• A same-day Baku round trip. 340 km each way for one bath is a long day in a car and very little else. If the trip is worth making, it is worth two nights.
• Booking off a star rating you saw on an agent site. Three Naftalan properties are sold two full stars above their official category. Check the register first; it takes a minute.
• A room-only rate with treatment bought on arrival. Properties bundle room and treatment differently, and Gold is the rare one publishing per-procedure prices. The arithmetic only works once you have both halves in writing.
• Planning a domestic flight leg. As of 11 August 2026, Nakhchivan was the only domestic departure on the Baku board. Anything you read about scheduled Ganja or Gabala flights probably dates from 2017.
• Paying a deposit before the conversation with your own doctor. We will hold a room while you have it. We will not push the deposit through ahead of it, whatever the property's availability pressure says.

What we don't claim

Three limits on what this page is. They are not disclaimers bolted on at the end; they are the reason we can publish property counts, star categories and road distances with any confidence at all. Where the answer is clinical, the honest response is that it is not ours to give.

• We don't call naftalan oil a cure. The evidence supports "traditionally used for" and "reported to help" language, not "treats" or "cures."
• We don't quote a single all-in medical price. Naftalan programs bundle room rate and treatment differently by property; we send a current quote once we know your dates and any medical restrictions.
• We don't handle the clinical side. Diagnosis, contraindication screening, and course design are the sanatorium's clinical team's job, not ours.
